This is a fairly easy itinerary but with some challenges.
Going by land from the Omo district in SW Ethiopia into the Lake Turkana area in Kenya seem to be a situation where you check out of Ethiopia maybe already in Jinka and check into Kenya in Lodwar. There doesn't seem to be a border post. Does anybody know?
The next area is getting from Gombe Nat Park in Tanzania to Lake Kivu in Rwanda.

You made no mention of other parts of Ethiopia and unless you have covered the rest of the country on previous trips, it would be sad to see you miss what this amazing country has to offer. I have traveled to much of Africa and Ethiopia is one of my favorite countries. If you want more details, just let me know.
There are several reasons for this particular itinerary. Yes, my first time in Ethiopia was in 1974 (in July when Heile Sellasie was killed, I was in Tendaho outside Dessie). Omo is where I never went. Yes, I admire the country and will be back for more incl Somaliland.
I was in Ethiopia in 2018/9. I travelled around the Omo Valley and then to Harar and Jijiga and across the border into Somaliland and Hargeisa. At the time, that was a safer crossing than going overland into Kenya from Ethiopia. The Kenya/Ethiopia border was as good as closed then. I suggest you check with someone who might have been there more recently to see if this crossing is now viable.

The official Ethiopia-Kenya crossing is at Moyale. Suggest you think carefully about crossing at any other point along the border for safety reasons. I speak from experience having traveled extensively in Africa and other regions of the world, including several troubled lands. Another factor to consider: Both countries require visas to enter for citizens of many countries, including yours. Typically there's an entry stamp when you enter, and an exit stamp when you leave. Not having an entry stamp in your passport when you try to leave can be problematic. Kenya now has an ETA program. Although I have not traveled recently in Ethiopia, I was there in both 2013 and 2014 (see my Travellerspoint maps). I traveled mostly by road, hiring the same driver both years. Total travel time in Ethiopia: Two months.
